Per-campaign fee coverage defaults

Override the org-wide fee coverage default on a per-campaign basis. Some campaigns (year-end, board appeals) work better with fee coverage on by default; others (silent auctions, event ticketing) work better with it off. Give every campaign its own default while respecting the donor's right to change it. A small flexibility that significantly improves conversion on the right campaigns.
